Lone Star Region USAV Membership – 2007/2008
Season
Texas Revolution Volleyball is a
member club of the Lone Star Region and the United States Volleyball
Association. Each player on a TRV team must also be a member of USAV. ALL
MEMBERS must fill
out the form and register with USAV in order to play. The region must have our player registrations before the
players can participate in a sanctioned tournament.
In order to successfully register, please do the following:
- Go
to http://lsvolleyball.org, and select Registration.
- Based
on the prompt:
- Select
“new member” if you did not play for any club in 06/07.
- Select
“previously registered online” if you played last season.
- Completely
fill out the online membership form and select “Continue.”
- In
the Club Drop Down Menu select “Texas
Revolution Volleyball,” then enter your Parent’s Name and Email
Address then select “Submit.”
- Confirm
that all of your information is correct then Click the “I agree” button
and then confirm. (Note: it is very important to check and make sure that
you have selected Texas Revolution
Volleyball as the club. If you put another club then they will have
to release the player and then you will have to register again).
- Please
print the waiver/code of conduct form.
- We
would like to have a copy on file for the region office, just in case
they need it.
- An
electronic signature (Parents Drivers Licenses) will have to be
entered. By doing such you
have agreed that the electronic signature is yours and agree to the terms
of the agreement.
- Please
print the USAV medical waver and bring that to any Sunday session.
- The
USAV registration fee is $30, and you can make the check out to TRV. The
club will then send the Region one check to cover all of the players and
coaches.
If you are having trouble with your USAV online registration
(forgot your username/ password) you must call the USAV Lone Star Region
